POSITION SUMMARY: The Bereavement Coordinator provides supportive services to help meet the needs of the terminally ill hospice patients and families as needed. Assists and support to families during their bereavement. Works as a member of the hospice team in providing comprehensive end-of-life care. Responsible for developing, implementing, and supervising the bereavement program and for the delivery of related services.


ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

• Manages the bereavement services program utilizing professional staff and volunteers.
  
  

• Oversees adequacy and appropriateness of bereavement programs for patients and family members.
  
  

• Develops new bereavement programs and services as needed.
  
  

• Develops educational programs and materials for patients, families, staff, and the community regarding loss, grief, and coping with bereavement.
  
  

• Assists agency leadership in educational training program.
  
  

• Designs materials for distribution to families eligible for and/or receiving hospice bereavement services.
  
  

• Plans, implements, and supervises bereavement group events.
  
  

• Supervises support staff involved in bereavement program.
  
  

• Oversees bereavement follow-up by patient care staff and volunteers.
  
  

• Oversees the volunteer component of the bereavement services.
  
  

• Conducts the bereavement section of the IDG conference.
  
  

• Participates in the maintenance of the bereavement component of the community and staff resource libraries.
  
  

• Participates in the orientation and training of new employees and volunteers working in the hospice program.
  
  

• Prepares reports as requested by agency leadership.
  
  

• Attends Interdisciplinary Group (IDG) conferences as required.
  
  

• Works with IDG concept of patient care.
  
  

• Other duties as assigned by agency leadership.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION:

• Graduate from an accredited institution of social work, preferred.
  
  

• At least two (2) years of experience in a healthcare or hospice setting, preferred.
  
  

• Must have experience or education in grief or loss counseling. Active patient contact within past three (3) years, preferred.
  
  

• The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver's license, maintain automobile insurance coverage and have access to an automobile.
  
  
• LCSW REQUIRED
  

PREFERRED KNOWLEDGE AND SKILLS:

• Must have strong computer skills to utilize Microsoft Outlook and agency systems.
  
  

• Self-motivated; able to work independently with minimal oversight.
  
  

• Demonstrated ability to supervise and direct others as needed.
  
  

• Ability to respond appropriately to crisis and immediate patent/family needs.
  
  

• Acceptance of and adaptability to different social, racial, cultural and religious modes.
