Resident Care Manager

Our CORE Values are Compassion, Acceptance, Respect, Dignity Position Summary The Resident Care Manager is responsible for coordinating and overseeing the daily delivery of resident care services at Francis House. This role ensures that residents receive compassionate, person-centered support consistent with the organization’s mission and standards of care. The Resident Care Manager works closely with caregivers, volunteers, families, hospice partners, and the interdisciplinary team to promote comfort, dignity, and quality of life for each resident. Essential Job Duties and Responsibilities (Additional duties may be assigned) Resident Care Coordination Oversee day-to-day-to-day resident care operations and ensure continuity of care across shifts. Coordinate admissions, resident transitions, and care planning in collaboration with hospice and healthcare partners. Monitor resident needs and advocate for timely communication of changes in condition to the appropriate clinical staff. Maintain accurate and confidential resident documentation in compliance with organizational and regulatory requirements. Prepare resident medications for administration by un-licensed staff as outlined by NYS guidelines. Will share responsibility as back-up to the On-Call Administrators. Team Leadership and Support Provide supervision, guidance, and support to caregivers and resident care staff. Assist with staffing coordination, scheduling, and shift coverage as needed. Participate in onboarding, orientation, training, evaluating and ongoing development of staff and volunteers related to resident care practices. Foster a collaborative, respectful, and mission-driven team environment. Off-shift volunteer support. Family and Resident Support Promote a welcoming, compassionate environment that honors resident preferences, culture, and dignity. Support families through communication, education, and coordination with community and hospice resources. Serve as a point of contact for residents and families regarding care coordination and support services. Evaluate effectiveness of plan of care and advocate for changes as needed. Provide follow-up phone calls to potential residents. Quality and Compliance Ensure adherence to Francis House policies, safety procedures, and applicable regulations. Participate in quality improvement initiatives, incident review, and problem-solving related to resident care. Monitor supplies and equipment needed for resident care and coordinate replenishment as appropriate. Education and Formation Responsibilities Provide on-going caregiver education. Mentor caregivers in sacred caregiving. Support caregiver resilience. Teach relational caregiving. Supervisory Responsibilities This position supervises the Scheduling and Orientation Associate, Caregivers, and On-Call Administrators. Knowledge, Skills, and Abilities Strong understanding of hospice and palliative care principles including patient care charts and histories. Excellent communication, interpersonal, and conflict-resolution skills. Ability to lead and support a multidisciplinary team in a compassionate residential environment. Strong organizational skills and attention to detail. Ability to maintain confidentiality and exercise sound judgment. Basic computer skills including the ability to send/receive e-mail messages, perform basic word processing and/or data entry, and proficiency with electronic documentation. Ability to respect the dignity of all who serve and are served. Ability to represent Francis House effectively in a variety of settings and with diverse communities. Ability to educate residents and/or families as to the nature of disease and to provide instruction on proper care and treatment. Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. Ability to observe, assess, and record symptoms, reactions, and progress. Ability to develop, implement, and modify multidisciplinary healthcare plans. Ability to make administrative/procedural decisions and judgments. Knowledge of clinical and social services documentation procedures and standards. Equipment Used Computer, printer, photocopier, telephone, fax machine, calculator, scanner and a variety of medical equipment and instruments including, but not limited to Hoyer lift, stretcher bed and wheelchair. Computer Software: Microsoft word, Outlook, PowerPoint and Excel, Paylocity Payroll Software. Safety Equipment: Work environment involves exposure to potentially infectious diseases and unpleasant tasks that require the use of latex or vinyl protective gloves, gown and mask. Education and Experience Bachelor’s degree in nursing, RN licensure required. Minimum of 3 years of experience in hospice, palliative care, long-term care, or a related healthcare setting. Prior supervisory or care coordination experience preferred. Physical and Mental Requirements Ability to sit, stand, walk, talk, hear, use hands to finger, grasp, handle or feel, and perform repetitive motions of the hands and/or wrists; push, pull, reach, stoop, bend, kneel and climb. Exerts up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently or constantly to move objects. Close mental and visual attention required to perform machine operations and set up, read, and document resident information. Environmental Conditions Work is performed in a typical medical environment. Duties include handling garbage or soiled linen. There is substantial exposure to blood, body fluids and/or pathogens, infectious diseases, unpleasant odors and hazardous materials that require following extensive safety precautions and include the use of protective equipment.
