Nurse Practitioner
Project H.O.P.E.
Duties The nurse practitioner is a Masters’ prepared independently licensed nurse who provides medical evaluation and care to patients at Project HOPE team. Operate independently utilizing an evidence-based practice model. Assess health status of patient through the collection of health data, physical assessment, interpretation of lab results and other diagnostic tools. Assessment will include consideration of biophysical, psychological, developmental, social, functional environmental and educational needs of patients. Disseminate educational information to better empower all staff as needed. As part of a committee or team, develop protocol and guidelines for patient retention, frequency of visits, appropriate prescribing, and better language around recovery care, as needed. Work collaboratively in Team Based Care as a member in a patient centered interdisciplinary care model with other departments including Behavioral Health, Medical Assistants, Nursing, and support staff. Participates is Daily Huddles Participates in Care Team meetings as scheduled Communicates patient management strategies to members of the care team Improve patient outcomes and delivery of care through clinical quality processes. Participate as the provider on call in part of the on-call rotation that is split fairly and evenly across medical providers. Responsible for following standards of care as identified by Project HOPE, NCQA, Guidelines of Family Practice, state and federal regulatory agencies. Accountable for a safe environment of care. Ability to engage in crucial conversations, offer constructive criticism, and receive constructive criticism. Collaborate with other departments to improve systems workflows and our quality metrics. Maintain compliance with local, national and federal requirements for education and certifications. Meet productivity requirements for 20-minute visits of up to 21 visits per day. Treat Opiate Use Disorder as part of a multidisciplinary team working in a harm reduction model using Buprenorphine-naloxone, Vivitrol, and adjunct medications. Have, or willing to get, an X-waiver. Prescribe and administer medication according to joint protocols and New Jersey State Law. Competency in interpreting urine drug screen results, other labs, and visit data to formulate a holistic, comprehensive, individual plan of care for each patient, or willingness to gain competency in these matters. Trained in the art of family or adult practice and experience in managing acute and chronic conditions typically seen in primary care such as hypertension, hyperlipidemia, type 2 diabetes, anxiety, depression, asthma, muscle/joint aches and pains, common infections, and physicals. Engage in critical thinking to formulate diagnoses, differential diagnoses, determine appropriate lab work, perform necessary health screenings, and determine appropriate areas for referral if needed. Perform COVID testing, teaching, supportive treatment, and vaccinations. Order and interpret diagnostic tests to develop and implement treatment plans. Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities to support the health and comfort of patients. Assess patients to determine individual needs and develop care plans in coordination with multidisciplinary healthcare professionals. Educate patients on treatment options, medical needs, and non-medical options for treatments and therapy. Perform patient physicals and compare and contrast results with established medical records to provide proper diagnosis and facilitate appropriate treatment. Appropriately diagnose and treat a variety of diseases and injuries through careful questioning and assessment of patient symptoms. Discuss care plans with patients and explain test results and proper use of medications. Educate patients and caregivers on healthcare protocols and processes. Work with families, caregivers, and patients to determine best options for treatment, diagnosis and prevention. Adhere to all HIPAA regulations. Support patients in maintaining prescribed regimen of care. Refer patients for consultation when indicated i.e. behavioral health, dermatology, neurology, ophthalmology, endocrine, infectious disease, psychiatry, social services. Records & documents assessment data, interventions, results and patient outcomes. Document in-depth progress notes and correct billing and diagnostic codes. Participates in daily huddle and care management meeting Obtains accurate histories from patient, family or significant other. Assesses & Manages follow-up plans. Educates patients, family and/or significant other in order to promote wellness, prevent health problems, maintain current health and intervene in acute or chronic illness. Participate as a team member in new and ongoing projects Assist the Medical Director in program evaluation and perform special projects as requested As requested, provide supervision of nonprofessional and professional personnel. Assist in the development of competency programs and customer services initiatives. Assessing own performance status in relation to this position description and progress toward achievement of professional goals on an annual basis Completes all required documentation for patient billing purposes accurately with 24 hours Participate in Quality Assurance and Quality Improvement Activities; Instructs nurse practitioner students rotating through Project HOPE Maintaining a portfolio of continuing professional development (CPD) activities. Perform other related duties as assigned. 1. Ability to quickly learn and adapt to a new Electronic Health Record with an inquisitive, can-do attitude. 2. Comfort in mobility. Our environment is ever-changing. With expanding and constricting staffing levels so space is at a premium. At times you will have a space at the clinical station, at times you may be asked to work out of an exam room. You may have a locker if there are lockers available. There will likely not be a formal designated office space. 3. Comfort in working in a diverse environment with a goal of patient and staff equity. 4. Situational awareness of the fact that this facility treats an underserved population whose life experiences may be very different from our own, and that our goal is to provide the highest level of case available for each individual patient. 5. Situational awareness that each role provided at Project HOPE is of equal importance as your own. Have a balanced inner-self, good tools for personal stress reduction, and a personal tool kit to support long-term growth and success, and reduce personal risk of burnout. Knowledge, Skills and Abilities: 1. Knowledge of federal, state, and other applicable standards for clinical practice. 2. Ability to provide leadership/guidance in a supervisory role and the flexibility to function as a team member at the management level. 3. Excellent managerial & supervisory skills with the ability to plan/organize and follow through in a timely manner. 4. Ability to work collaboratively with diverse individuals and situations, including strong problem solving and conflict resolution skills. 5. Excellent innovation skills; requires development of new protocols, policies, and procedures for clinic function. 6. Proven understanding of quality improvement initiatives and collaboratives. 7. Experience dealing with underserved populations and cultural competency. 8. Ability to understand and respond appropriately, effectively, and sensitively to special population groups served by ICHC. Special population groups include those defined by race, ethnicity, language, age, sex, sexual orientation, economic standing, etc. 9. Ability to travel for training, collaboration with other organizations & agencies, oversight of satellite clinics, and to other areas of community as necessary. Education, Licensing and Experience: 1. Master’s degree in Nursing. 2. Valid RN License in the state of New Jersey or ability to obtain a license in New Jersey. 4. BLS certification 5. Valid DEA and CDS licenses. 6. Preferred experience in an outpatient health clinic setting caring for underserved populations. 7. Valid driver’s license. 8. Minimum 1 years’ experience as a nurse practitioner 9. Excellent communication skills, collaborative interpersonal skills, and an outstanding record in clinical care and education. 10. Experience with working in Electronic Health Records (EHR) 11.
