Organization Overview Covenant House California (CHC) is a nonprofit organization that provides sanctuary and support for youth overcoming homelessness, ages 18‑24. Position Summary Under the supervision of the Director of Community Housing and Alumni Support, the THP‑Plus Case Manager evaluates, places, and assesses prospective and enrolled eligible young adults participating in the Transitional Housing Plus Program (THP‑Plus). The target demographic includes young adults (18‑24) who have been emancipated from foster care, including those supervised by Juvenile Probation. The program offers housing and supportive services in a safe living environment and helps participants develop life skills needed for independent living. Program participants are eligible for up to 36 cumulative months or until the age of 25, whichever comes first. Key responsibilities include ensuring the success of youth in securing stable housing, employment, education, well‑being, and supportive networks. Support services may include transportation assistance, development of daily‑living skills, counseling and mental‑health services, and obtaining vital documents. All services are delivered in accordance with the Mission Statement, Five Principles, and the Policies and Procedures of Covenant House California. Essential Functions Provide high‑quality support and develop positive relationships with residents and off‑site clients via the Covenant House California case‑management system. Initial Intake – Welcome youth to the program and review intake documentation completed by the receiving party. Full Intake Assessment – Complete the Intake/Assessment within 3‑7 days and assist youth in identifying and accessing resources. Individual Service Plan (ISP) – Develop and implement the Housing Plan within 30 days of intake, update every 30 days in HMIS and ETO systems; complete a Wellness Survey every 30 days; conduct regular home visits; maintain a calendar of work schedules and appointments; develop community resource connections; make appropriate referrals within CHC and to community agencies; engage in positive transition planning. Transition Plan – Assist youth in locating and visiting housing options, filling out applications, evaluating housing habitability, mediating with landlords and neighbors, providing information on tenant rights and responsibilities, reviewing lease requirements, correcting rental and credit histories, building household budgets, renegotiating debt, and providing employment and service referrals. Maintain accurate, timely documentation – Monitor progress of case plans, record case notes at least weekly, monitor program outputs, ensure external grant documentation, and record critical incidents, wellness surveys, engagement touchpoints, and follow‑up forms. Attend Department Case Review Meetings – Prepare and present assigned cases; participate in weekly check‑ins and case‑management meetings with supervisor. Group Facilitation – Facilitate or support group activities and case‑management meetings, ensuring safety and cleanliness of living spaces; maintain punctuality for all scheduled meetings and outdoor collaborative meetings; complete required trainings from CHC, CHI, CTA, LAHSA, DCFS, and others; adhere to CHC policies and the Interim Housing Scope of Required Services. Support additional duties – Cleanliness and safety of the living environment, crisis intervention, backup support for other direct‑care positions, and other tasks assigned by the Director. Job Requirements Education : Master’s degree in behavioral science or a graduate program approved by the California Private Post‑Secondary and Vocational Education Bureau. Experience : At least one year of administrative or supervisory experience over social work, childcare, or support staff providing direct services to children; or at least two years as a social worker in an agency serving children or a group residential program for children. Experience in a public or private child‑welfare social‑services setting, or with transition‑age youth (16‑24) is preferred. Prior experience with youth experiencing homelessness, mental illness, or system involvement, Title 22 regulations, and program development leadership is a bonus. Knowledge : Housing First model, Trauma‑Informed Care, harm‑reduction, positive youth development, adolescent development, case management, crisis intervention, community resources, and sensitivity to homelessness and human trafficking. Skills : Self‑starter, team player, excellent written and verbal communication, public speaking, independence, organizational and time‑management abilities, computer skills (Word, Excel, email, calendar, database operations). Bilingual English/Spanish strongly preferred. Transportation : Valid California driver’s license, reliable vehicle, good driving record; may be required to meet clients off‑site or provide transportation for residents and staff. Working Conditions : Office and communal residential environments; may need to lift up to 40 lbs, bend, sit, and stand for prolonged periods, and navigate stairs. Weather may vary; noise level moderate to loud. Reasonable accommodation provided for disabilities. Schedule : Full‑time, 40 hrs/week; evening, weekend, and holiday work may be required. Supervisory Duties : This position does not perform supervisory duties, though it may facilitate team meetings or provide training to other team members.
