Care Management Supervisor - Utilization & Discharge
Hackensack Meridian Health
Hackensack Meridian Health is seeking a Supervisor, Care Management, on-site at Jersey Shore University Medical Center (JSUMC). This role oversees utilization management, care coordination, discharge planning and performance improvement for the care-management team.
The candidate will supervise staff, optimize LOS and patient flow, and ensure compliance with regulatory and payor guidelines while advancing care quality. Monday–Friday 8:00am–4:30pm, on-site at JSUMC.
