Home Health Clinical Manager
Bridge Home Health & Hospice
Bridge Home Health & Hospice is seeking a Clinical Manager to coordinate and supervise patient care in a home health setting. You will review referrals, assign clinicians, and ensure services are delivered effectively while maintaining high-quality patient outcomes. You will lead clinical staff, participate in case conferences, and support staff development, policy implementation, and community outreach as needed.
