DocStar Medical Partners - Job Opportunity, New Listing Facility City & State: Near Eureka, CA Facility Type: Outpatient FQHC Provider Profession Needed: FNP Specialty: Family Medicine Start Date: 8/3/2026 End Date: 11/5/2026 Length of Assignment: 3 months, option to extend additional 3 months Credentialing Timeframe: 2 weeks Work Schedule: M-TH, 8A-5:30P, 30min unpaid lunch Patient Volume: 18-20 PPD Procedures: Basic family medicine- wellness exam, cold, cough, flu, chronic care, diabetes, weight management. Bread and butter PC. Ages 6 months+ EMR: OCHIN Epic License Needed: CA Certifications Needed: Medi-Cal, DEA, BLS Relevant Experience/Skill Set Needed: Rural health Other additional pertinent information: Please include CA license number and NPI with submission. 2 other PAs in clinic. Will accept flu declination but provider would be required to wear mask at all times while in clinic. Must have Hep B, TB & Tdap titers, no exceptions
