Position Overview

The Nurse Practitioner will provide comprehensive primary care services to geriatric patients in multiple care settings. Responsibilities include conducting assessments, diagnosing and managing acute and chronic conditions, coordinating care plans, and collaborating with interdisciplinary healthcare teams to ensure continuity of care.

This role also includes participation in telehealth visits as needed and attendance at weekly virtual provider meetings.
Key Responsibilities

• Conduct comprehensive physical assessments and evaluations for geriatric patients
• Diagnose and treat acute and chronic medical conditions
• Develop and implement individualized treatment and care plans
• Manage chronic illnesses with a focus on reducing unnecessary hospitalizations and improving quality outcomes
• Provide preventive care, medication management, and patient/family education
• Coordinate care with nurses, therapists, specialists, social workers, and facility staff
• Document patient encounters accurately within eClinical Works EMR
• Participate in telehealth appointments when appropriate
• Maintain compliance with state and federal healthcare regulations
• Attend weekly provider meetings via Microsoft Teams

Patient Care Environment

Providers will care for patients in:

• Skilled Nursing Facilities (SNFs)
• Assisted Living Facilities (ALFs)
• Private residences/home visit settings

This is a community-based role requiring strong time management, independent clinical judgment, and excellent interpersonal communication skills.
Productivity & Earning Potential

• Base salary of $100,000 for meeting standard patient expectations
• Home visit production goal of approximately 9 visits per day
• Lucrative productivity bonus structure with significant upside potential
• Approximately $7,000 in additional annual income for adding just one additional patient per day
• Typical total earnings range from $120,000-$160,000 annually
• Top-performing providers can earn approximately $200,000 annually.

Requirements

Required

• Active Florida APRN license
• Active DEA license
• Strong clinical assessment and communication skills
• Ability to work independently in community-based environments

Preferred

• Prior geriatric care or primary care experience
• Experience using eClinical Works EMR
