About the Role Talin Home Healthcare LLC is looking for multiple compassionate and dedicated Home Health Nurses to serve patients across different Northern Virginia counties area. Responsibilities Provide skilled nursing care to patients in their homes across the Northern Virginia area. Conduct comprehensive patient assessments and develop individualized care plans. Administer medications, wound care, IV therapy, and other clinical treatments as ordered. Monitor and document patient progress, vital signs, and changes in condition. Educate patients and family members on disease management, medications, and home care techniques. Collaborate with physicians, therapists, and care team members to ensure coordinated patient care. Maintain accurate and timely clinical documentation in compliance with regulatory standards. Requirements Active RN license in the state of Virginia. Oasis and SOC Experience preferred. Previous home health, med-surg, or community nursing experience preferred. Strong clinical assessment and critical thinking skills. Excellent communication and patient education abilities. Reliable transportation and valid driver's license for travel throughout the NOVA region. Ability to work independently and manage a flexible patient caseload. Compassionate, patient-centered approach to care. Must commit to working a minimum of 3 days a week Mon/Wed/Friday.
