Outpatient Physical Therapist

Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA is seeking a Physical Therapist to join our Outpatient Physical Therapy team to join our team! 

Full-time position available with complete benefits package including Medical, Dental, Vision, Paid Time Off, 403B with company match, and more. Hours typically 8AM to 4:30PM but may require some rotating weekends. 

PT shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician.

• Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.
• Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
• Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.
• Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
• Identify needs for program development and implement programs according to company policies and procedures.
• Complete all required documentation according to guidelines specified in Documentation Manual.
• Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents.
• Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month.

Requirements:

• Must have a degree in Physical Therapy from an accredited program.
• Active state license required.
• Excellent organizational and time management skills.
