What You'll Do Evaluate residents and develop individualized treatment plans Provide evidence-based physical therapy interventions Improve strength, balance, mobility, endurance, and functional independence Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families Assist with discharge planning and safe transitions home Educate residents, families, and caregivers on mobility and safety Maintain accurate and timely documentation Participate in interdisciplinary care planning meetings Help create a positive, motivating therapy environment where residents look forward to treatment What We're Looking For Current Physical Therapist license in the State of Colorado Graduate of an accredited Physical Therapy program Strong clinical reasoning and assessment skills Excellent communication and teamwork abilities Passion for improving the lives of older adults Skilled Nursing Facility experience preferred, but not required New graduates encouraged to apply Why You'll Love Working Here Competitive Pay $50–$60 per hour Flexible Scheduling Full-Time and Part-Time opportunities available.
