Portland Community Reinvestment Initiatives, Inc. is a non-profit community development corporation with a mission “to preserve, expand and manage affordable housing in the City of Portland and provide access to and advocacy for services to our residents”. With over 800 units of affordable housing, PCRI’s unique mix of single-family homes, small multi-plexes and community apartments represents one of the last stable opportunities for low-income households to remain in their vibrant Portland neighborhoods. These homes and apartments are woven into the fabric of their neighborhoods and are a model for eliminating concentrations of poverty.

To accomplish PCRI’s goal of addressing involuntary displacement, PCRI established a displacement mitigation initiative, Pathway 1000, with the sole purpose and intent of slowing and reversing the involuntary displacement of long-term residents previously forced to move from N/NE Portland, and current residents at risk of displacement. Through the Pathway 1000 initiative, PCRI aims to build and create at least 1,000 homes in the next 10 years, many of which will be available to purchase. The homes will be located throughout the city of Portland, with the primary focus on the N/NE Portland neighborhoods where displaced families previously resided.

Position Summary:
The PSH Support Specialist provides support to a diverse group of residents living in Multi-Family Community Apartment Building & other PCRI affordable housing sites. The PSH Support Specialist will work with residents directly as the manager of each case and complete all compliance and reporting related tasks associated with them. This position requires a motivated and resourceful individual who is sensitive to resident needs; has excellent service coordination, team building, and group facilitation skills; and can thrive in a relatively independent environment working with a multidisciplinary team. This position will include working evening, early morning and weekend shifts.

Essential Functions: 

• Assist in maintaining personnel files and case management for residents of Kafoury Court. Work collaboratively and respectfully with clients to develop effective housing plans to address immediate and long-term housing and healthcare needs.
• Provide individualized and tenant-driven case management services to residents.
• Develop and implement personalized service plans that address residents' needs and goals related to housing stability, mental health, and overall well-being.
• Maintain timely, accurate, and appropriate documentation of services in Multnomah County’s Homeless Management Information System (HMIS).
• Track resident outcomes for reporting and compliance 
• Conduct regular check-ins with residents to monitor progress and adjust service plans as needed.
• Assist residents in maintaining long-term housing stability by providing continuous support and resources.
• Facilitate connections to community resources, such as mental health services, employment assistance, and other supportive services.
• Help residents transition away from intensive case management and/or rental subsidies when appropriate, supporting their "graduation" from the program.
• Coordinate with other service providers and programs to ensure residents receive comprehensive support.
• Refer households to this program if they are already receiving housing support through alternative programs but require continuous case management services.
• Work closely with the Housing Development Department (HD) and other stakeholders to ensure seamless service delivery.
• Adhere to all guiding principles and expectations outlined by Multnomah County, Oregon Housing & Community Services, and relevant funding sources.
• Maintain accurate and up-to-date case notes, service plans, and other required documentation.
• Ensure compliance with funding restrictions and program requirements.
• Support PCRI organizational objectives and goals as directed.
• Additional duties as assigned by supervisor or Executive Director

Job Scope:
The PSH Support Specialist reports to the Director of Housing Development. This full-time position at 40 hrs. per week; flexible hours will be required, and the position will include early morning, evening or weekend hours. The PSH Support Specialist will conduct regular check ins with residents and provide case management for residents in the community in order to assist them in maintaining their housing and quality of life. This position also works to enhance the sense of community and pride at the organization.
 
Education and/or Experience: 

• Bachelor’s Degree in Social Work, Psychology, or related field.
• Experience working with unhoused individuals and/or individuals with recognized challenges that may include: severe and persistent mental illness, substance abuse, social anxieties, etc. 
• Case Management experience preferred

Preferred Qualifications:

• Lived experience to be able to provide ‘Peer Support’ is preferred but not necessary
• Strong organizational and communication skills
• Ability to maintain confidentiality
• Ability to utilize basic computer software (Office Suite, Google, HMIS, Yardi, etc.)
• Reliable transportation is required
• Bilingual preferred

Supervisory Responsibilities:
This job has no supervisory responsibilities.

Certificates, Licenses, Registrations:
Licensed Clinical Social Worker (LCSW) Preferred.
 
Working Conditions:
This job operates in an office setting. This role routinely uses standard office equipment. Office hours are Monday through Friday 8:00am-4:30pm. Setting includes residential environment that houses individuals experiencing sever mental health or housing difficulties.
 
Compensation / Benefits:
Salary is commensurate with qualifications and experience. PCRI provides a generous benefits package that includes 10 days of paid Vacation after the completion of one year of employment that increase over time to 20 paid days. In addition to paid vacation, PCRI’s annual benefits package includes 9 days of paid Sick Leave and 3 days of paid Personal Time Off, 11 paid holidays, employer-paid health coverage (medical, dental, vision) that requires a contribution for employee if dependents added to plan, Long Term Disability Insurance, 403(B) employer match contribution of up to 3% of employee’s annual salary. Voluntary benefits include: Life Insurance, Short Term Disability insurance, critical and accidental insurance.

You may have a review at 30, 60, & 90 days but these reviews do not have compensation increases attached to them.

Physical Requirements:

• Must be able to regularly walk up and down stairs up to 8 hours per day.
• Must have the ability to stoop, twist, reach, and pull.
• Must be able to sit and/or stand for up to 8 hours a day.
• The worker is required to have close visual acuity to perform an activity such as viewing a computer terminal and extensive reading.
• Ability to lift up to 20 pounds, unassisted.
• Ability to assist in physical set-up activities (e.g., setting up and rearranging folding tables and chairs, etc.)
