Contract | 13-Week Assignment | Day Shift (5×8, Monday-Friday, 8:00 AM-5:00 PM) | Weekend rotation may be required (frequency varies by location - discuss during interview) | On-call rotation may be required (discuss with manager during interview)

Key Responsibilities

• Provide home health physical therapy services including Start of Care and follow-up visits to a caseload of approximately 5-10 patients per day
• Travel independently to patient homes daily within the assigned territory, covering multiple counties; must be willing to travel up to 75 miles from the home clinic
• Attend periodic office/clinic meetings as required
• Meet weekly productivity expectations of approximately 30-32 points per week, with each visit type and activity (documentation, travel, etc.) assigned a point value
• Complete all patient documentation within 24 hours of each appointment
• Maintain current driver's license, reliable personal vehicle, and agency-required auto liability insurance

• Active, unencumbered Physical Therapist license in hand at time of start (pending licenses are not accepted, except where licensure issuance is delayed by only a few days)
• Minimum 1 full year of recent Adult Home Health experience required; first-time home health applicants will not be considered
• CPR/BLS certification required
• Proficiency in HomeCare HomeBase (HCHB) EMR; must have used it within the last 6 months or be willing to complete a retraining course during compliance
• Must accept all patients assigned to daily schedule regardless of distance from previous appointment
• Must be open to on-call rotations and traveling up to 75 miles from the assigned care center, which may result in drives longer than 20 minutes between patients
• Local candidates are welcome and accepted at the same rate

• Experience with Start of Care assessments and OASIS documentation in a home health setting
• Familiarity with multi-county territory coverage and high-volume patient scheduling
• Experience working independently with minimal supervision in a field-based environment

• Mileage reimbursed at the IRS standard rate (patient home to patient home); if patient census is low, there may be an opportunity to float to a nearby clinic to maintain hours
• Opportunity to work autonomously in a dynamic home health environment serving patients across a multi-county territory
