Job Description
Skilled Home Health Therapist

Schedule

Day shift with block scheduling. 1-2 weekend shifts per contract; 6-8 skilled home visits per day (minimum of 6 points per day).

Assignment Details

Home health setting with an interdisciplinary support team.

Requirements

• Minimum of 1 year of home health experience required.
• OASIS training preferred.
• Current state license (Florida license or compact license) required at the time of submission.
• BLS certification required.
• Valid driver's license and reliable vehicle for home visits.
• Experience with home health documentation; OASIS experience preferred.

Responsibilities

• Conduct assessments and provide cardiopulmonary care.
• Deliver orthopedics and neurological care, including gait training and assistive device assessments.
• Implement home exercise programs and ADL training.
• Provide trach care and care for prosthetics/amputations.
• Educate patients and families on safety/injury prevention and medication understanding.
• Document care in electronic health records as per facility guidelines.

Compliance / Notes

COVID-19 vaccine required (medical/religious exemptions allowed with form). Influenza vaccine or declination required; mask must be worn while at work. Candidates must reside at least 100 miles from the facility.
