Description

• Assess patients in both inpatient and outpatient environments to develop personalized treatment plans
• Implement physical therapy interventions aimed at improving mobility, strength, equilibrium, and overall function
• Work in collaboration with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and exercises to do at home
• Maintain accurate records of evaluations, treatments, and progress as per facility standards
• Engage in interdisciplinary care team discussions to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications, such as those in geriatrics or neurology, are advantageous but not mandatory

Skills

• Prior experience is strongly preferred
• Ability to manage patients with complex medical and mobility challenges
• Excellent teamwork and communication abilities
