Evaluate residents and develop individualized treatment plans to enhance mobility, strength, balance, and independence. Provide skilled therapy interventions, monitor progress, and adjust treatments accordingly. Assess and recommend assistive devices and safety modifications. Educate residents, families, and caregivers on treatment plans and discharge strategies. Maintain accurate documentation and participate in interdisciplinary care planning. Ensure compliance with regulatory and professional standards. Ensure compliance with regulatory and professional standards. Qualifications And Skills Current Colorado Physical Therapist license in good standing. Strong clinical assessment, treatment planning, and communication skills. Ability to work independently and collaboratively within a team. Experience in skilled nursing, rehabilitation, or long-term care preferred. Knowledge of Medicare and managed care rehabilitation services is a plus.
