Primary Care Nurse

Job Overview

We have an exciting opportunity for a Primary Care Nurse to join our Primary Care Team at HMP Risley , a category C Prison located in Warrington. HMP Risley is a male prison that has a population of 1000. As a Primary Care Nurse, you will work as part of a team of 24 based within the healthcare department and some wing work is required.

Hours and Shift Details

• Monday - Sunday service
• 37.5 hours per week
• 7:30am – 8:45pm
• Potentially night shifts may be required
• 12‑week rota

Salary

This full‑time position offers an annual salary between £34,428 and £41,659 per annum FTE, dependent on experience.

Responsibilities

• Work within a multi‑disciplinary team to deliver a nurse‑led service and prevent hospital admissions
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use skills and knowledge to deliver nurse‑led clinics to your patient group whilst being supported by a multi‑disciplinary team
• Provide outstanding evidence‑based care to our patients, seeking out, listening to and acting on their feedback to personalise care
• Deliver non‑judgemental care that makes a real difference
• Inspire excellence by forging strong links with partners and stakeholders
• Promote best practice, clinical supervision, evidence‑based care and continuous quality improvement, embedding a culture of shared learning
• Share knowledge, skills and expertise to ensure safe care and build strong multi‑disciplinary teams

Qualifications – Essential

• Registered nurse (Adult) – NMC
• Evidence of continued professional development
• Understanding of complexities of multi‑disciplinary and multi‑agency working
• A non‑judgemental and compassionate approach
• Excellent communication skills
