If so, apply today!Why Join the Elara Caring Mission?Work in a collaborative environment.Be rewarded with a unique opportunity to make a differenceCompetitive compensation packageTuition reimbursement for full-time staff and continuing education opportunities for all employees at no costOpportunities for advancementComprehensive insurance plans for medical, dental, and vision benefits401(K) with employer matchPaid time off, paid holidays, family, and pet bereavementPet insuranceAs a Hospice Registered Nurse Case Manager, you'll contribute to our success in the following ways:Completes routine and emergency "face to face" recertification visits with patients entering their 3rd benefit period or beyond, including admissions and transfers and extended length of stay.Records observations, treatments, and other pertinent information.Communicate with Medical Director, Attending Physician, branch leadership and IDG regarding findings from the visits and any measures to alleviate symptoms.Provides physical and emotional care to support the patient and family in period of crisis.Ability to triage consults by patient acuity.Performs bedside palliative care with patient/family educationEnsures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Hospice team.Prevents unwanted hospitalizations by meeting patient and family needs in the home.Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.Compliance with applicable laws and regulations with respect to Collaborative Agreements.Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation.Provides training and continuing education for staff.Assists in development of clinical practice guidelines/standards in support of quality palliative care.Adheres to the practice of confidentiality regarding patients, families, staff and the organization.Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG.Performs other duties/projects as assigned.What is Required?Master's degree in nursing and certification through a national Nurse Practitioner Licensing board.1+ years of clinical nursing experience1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferredPrior experience in Family Medicine, Geriatrics and psychiatry preferred.Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.Maintain active DEA certification if state allows prescription of controlled substances by APPs50% travel requiredAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipmentValid driver's license and insurance and reliable transportation to perform job tasksYou will report to the Clinical Team Manager.Equal Employment Opportunity: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
