As a Registered Nurse Supervisor, you will play a vital role in ensuring the highest level of care for our patients while promoting a positive and supportive work environment for our home care professionals.As a Registered Nurse Supervisor, you will be responsible for supervising a team of home care professionals, providing guidance and support to ensure the delivery of exceptional patient care. You will also be responsible for:Coordinating patient care activities to ensure that all patients receive the highest level of care and supportSupervising and mentoring our home care professionals to promote professional growth and developmentMaintaining effective communication with patients, families, and healthcare providers to ensure that all parties are informed of patient status and treatment plansIdentifying and addressing any concerns or issues related to patient care or staff performanceResponsibilities:Oversee patient care within the evening shift, ensuring that all patients receive timely and effective careSupervise and evaluate the performance of home care professionals, providing constructive feedback and coaching as neededCollaborate with other healthcare professionals to ensure that patients receive comprehensive and coordinated careMaintain accurate and timely records of patient care and staff performanceRequirements:Current RN licensure in the State of New YorkMinimum 2+ years of experience as a Registered Nurse, with experience in home care or long-term care preferredExcellent communication and leadership skills, with the ability to work effectively with patients, families, and healthcare professionalsStrong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilitiesWhy Join Our Team:At Constant Care Health Services LLC, we are committed to delivering exceptional patient care and creating a supportive and inclusive work environment.
