Housing-First Street Outreach Case Manager

Agate Housing and Services is seeking a Street Outreach Case Manager to work with single adults in Hennepin County. You will carry a housing-focused caseload, engage unsheltered individuals, and coordinate with programs to help participants reach stable housing. Role involves trauma-informed approaches, documentation in HMIS/ClientTrack, and collaboration with the Welcome Center and community partners.
