Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental function, aligned with department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Perform initial evaluations and ongoing assessments; develop and modify individualized PT treatment plans for the outpatient population.
- Implement targeted interventions (therapeutic exercise, manual therapy, gait and balance training, neuromuscular re-education) and monitor patient progress.
- Document progress, correlate with goals, and communicate updates to the multidisciplinary team; contribute to discharge planning as appropriate.
- Educate patients and families on home exercise programs, safety, and self-management strategies; ensure patient understanding and engagement.

Requirements
- Active physical therapy license in California or eligibility to obtain licensure.
- Doctor of Physical Therapy (DPT) or equivalent required by state standards.
- Experience in outpatient physical therapy preferred.
- Ability to work in an outpatient setting; daytime schedule with 5x8-hour shifts.
- Strong communication and collaboration skills to function within an interdisciplinary team.
