We are proud to be a mission-driven, values-based organization and a Certified Great Place to Work.What You'll DoAs a Nurse Care Manager, you will play a critical role in BHN's Complex Care Management (CCM) Program, helping individuals with complex medical, behavioral health, and substance use needs navigate healthcare systems and maintain stability in the community.
