POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Facilitates patient care across the continuum of an assigned number of cases by managing, coordinating, facilitating, and monitoring resources to achieve optimal clinical outcomes, utilization of services, and financial goals; Oversees the discharge process for all patients within the unit/core service area; Integral to the team's effort to establish a safe and appropriate discharge plan/plan of care; Possess a working knowledge of the spectrum of hospital and post-discharge services that are available.

MINIMUM REQUIRED QUALIFICATIONS – 

• NY RN license
• BLS
• 1.5+ years recent case management experience (within past 2 years) in an acute care facility of similar size and level as this facility
• Inpatient pediatric experience

PREFERRED QUALIFICATIONS – Epic experience

LENGTH OF ASSIGNMENT – 13-15 weeks

SHIFT / HOURS PER WEEK – 8hr days

SYSTEMS – Epic

START DATE – ASAP
