About the Role:
As an LPN at Concord Care of Hartford, you will play a vital role in delivering high-quality patient care to our residents. Your responsibilities will include:
Providing direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
Developing and implementing individualized care plans in collaboration with the interdisciplinary team
Participating in care conferences and contributing to the development of treatment plans
Maintaining accurate and detailed records of patient care and progress
Collaborating with other healthcare professionals to ensure seamless transitions and exceptional patient outcomes
Requirements:
Current LPN licensure in the state of Ohio
High school diploma or equivalent required
Previous experience as an LPN or related healthcare experience
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
