Registered Nurse

The RN utilizes an interpersonal process that focuses on the total individual and their role in their family, culture, and society. The RN utilizes the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. The RN fulfills a resident advocacy role by ensuring the resident and/or their representative has adequate information to make health decisions. They coordinate total care and work with the resident to achieve or maintain their highest practicable level of function. The RN is guided by specialized knowledge of nursing based on physiological and psychological sciences. They ensure that the quality of resident care meets legal and professional standards and acts as a role model to other members of the healthcare team. This position requires full understanding and active participation in fulfilling organizational mission, vision, and values. The RN is expected to demonstrate behavior consistent with core values and support the goals and direction of the quality improvement process. The RN completes holistic, age-specific admission history, physical assessment, and nursing plan of care on all assigned residents in an ongoing manner to monitor for deviations. They analyze and incorporate clinical data to formulate an individualized plan of care. The RN identifies and documents residents at risk for nutritional and functional status. They identify and report signs and symptoms of neglect and abuse. The RN sets priorities for and reviews daily plan of care in collaboration with the resident, family, significant other, and other healthcare disciplines. Delegation to team members is based on resident needs, continuity of care, staff, and knowledge. The RN communicates with physicians to ensure coordination of medical and nursing plans of care. They demonstrate competence in performing technical skills. The RN proactively recognizes and manages urgent and emergent resident care situations. They demonstrate knowledge and technical competence in preparation and administration of medications and IVs per scope of practice and policy. The RN documents nursing interventions, resident responses to care, and resident outcomes. They evaluate data to revise the plan of care to meet the changing needs of the resident. The RN evaluates responses to interventions against expected norms for age and development. They identify and document resident and family learning needs and provide education based on assessment in collaboration with the resident, family, and interdisciplinary team members. The RN identifies and documents discharge planning needs and coordinates or delegates discharge activities. They document resident and family responses to education. The RN maintains working knowledge of regulations to ensure documentation reflects compliance. They incorporate resident needs and preferences into organized, efficient delivery of care by a team assigned for daily care and delegate tasks appropriately. The RN supervises, coaches, counsels, and evaluates performance of assigned staff. Physical demands include lifting objects over 100 lbs. with frequent lifting and carrying up to 50 lbs. The RN is required to maintain skills that fulfill the demands of the position.
