Job Title: Registered Nurse Profession: Nursing Specialty: Medical Intensive Care Unit Duration: 26 weeks Shift: Night Hours per Shift: 12 hours Experience: 1 year minimum in ICU required License: Pending license acceptable Certifications: BLS (AHA), ACLS (AHA), NIH Must-Have: - 1 year recent ICU experience - Ability to manage patient acuity levels Description: We are offering a 26-week assignment. Holidays are required. No requests for time off in the first two weeks of the assignment. All requests for time off must be presented prior to the offer. If requests for time off are received after the offer, they must be discussed with management. A concrete estimated time of arrival is required at submission, along with proof of application. Licenses from Michigan are acceptable if pending. Patient ratios can be as follows: 1:1-4. Our unit cares for both PCU and ICU patients. PCU and general medical/transfer status patients can be managed in a group of 4:1 based on acuity. PCU patients can also be in a 4-3:1 ratio based on acuity. ICU and PCU patients can mix in a ratio of 3:1 based on acuity. Ventilator patients are typically in a 2:1 ratio. Cooling, continuous renal replacement therapy (CRRT), or high acuity patients will be in a 1:1 group. Weekend work is required, with a schedule of every other weekend. Nursing staff will float to other ICU and medical-surgical units as needed. Staff will rotate in floating to medical-surgical units based on census needs. Typical nurse-to-patient ratios in medical-surgical areas are approximately 1:5-7. Patient diagnoses include COPD, encephalopathy, drug overdose, DKA, acute kidney injury, respiratory failure, cardiac arrest, heart failure, and atrial fibrillation. Special procedures include CRRT, hemodialysis, arterial line placement, central venous catheter placement, tracheostomy placement, and chest tube placement. Special equipment used includes cooling machines, Bair Huggers, for massive transfusions, and CRRT. Required skills include cardiac monitoring and emergency response such as interpretation and management of dysrhythmias and multi-system organ failure. Experience with IV insulin protocols and management of acute and chronic renal failure is required. IV therapy experience should include skills in arterial line management, blood product administration, and central line care. Nursing staff will also manage continuous IV sedation and administration of emergency medications. Knowledge of neurological assessments for CVA and acute stroke management is essential. Respiratory care skills involve management of BiPAP, CPAP, and care of ventilated patients. Interpretation of arterial blood gases and suctioning techniques are required. On average, our unit has three ventilated patients per day. Patients may remain in ICU per physician discretion, even if acuity levels are low. In these cases, patients may be grouped with 3 or 4 other stable patients.
