- Provide peer support to individuals with substance use and co-occurring behavioral health concerns
- Meet with program participants to provide peer mentoring, linkages to resources, and facilitation of contacts with the recovery community
- Create participant-driven wellness plans designed to address the barriers to wellness, including; stigma, transportation, basic needs, housing and others as defined by the participant
- Provides outreach to the community and distributes literature, brochures, and other related materials in an effort to educate the general public to promote the programs and to reduce stigma
- Consistently document all participant encounters and outreach efforts
- Participate in, and contributes to, team meetings, providing constructive feedback to support the participants, the peer team and the peer programs
