Always Best Care is seeking an experienced Registered Nurse (RN) to join our clinical leadership team as a Clinical Director Designee. This position combines the hands-on responsibilities of a Clinical Supervisor with an important additional leadership responsibility: serving as the designated backup to the Clinical Director when the Clinical Director is unavailable. You’ll work closely with the Clinical Director to support our clinicians, oversee patient care, strengthen clinical operations, and maintain high standards for quality and compliance. This is not strictly a desk position. The Clinical Director Designee remains actively involved in patient care, supervisory visits, clinical problem-solving, and field coverage when needed. Why Join Our Team? * $125,000 annual salary * Key clinical leadership position within a growing home health agency * Work directly with and support the Clinical Director * Opportunity to develop experience in higher-level clinical operations and leadership * Mentor and support RNs, LPNs, therapists, and home health aides * Remain connected to patients and field clinicians * Help shape clinical processes, quality, compliance, and patient outcomes * Join a locally operated agency serving patients throughout Oahu What You’ll Do Clinical Leadership & Staff Support * Supervise, mentor, and support clinical staff * Serve as a clinical resource for RNs, LPNs, therapists, and home health aides * Provide clinical guidance and help resolve patient-care concerns * Assist with staff orientation, training, competencies, and ongoing development * Conduct supervisory visits and competency evaluations * Promote accountability, communication, and high standards of patient care Patient Care Oversight * Review patient assessments and plans of care * Ensure services are delivered according to provider orders and the established plan of care * Assist with complex, high-risk, or changing patient situations * Coordinate with physicians, providers, facilities, and interdisciplinary team members as appropriate * Perform patient visits, including SOCs, ROCs, recertifications, supervisory visits, skilled nursing visits, and other clinical visits as needed * Provide field coverage when necessary to maintain continuity of care Documentation & Compliance * Review OASIS and other clinical documentation for accuracy, completeness, and timeliness * Help ensure documentation supports medical necessity and skilled need * Monitor compliance with Medicare requirements, agency policies, and applicable regulatory standards * Identify documentation or clinical deficiencies and assist staff with corrections and education * Collaborate with QA staff on audits, trends, corrective actions, and performance improvement Quality Improvement & Clinical Operations * Participate in QAPI and performance improvement initiatives * Identify clinical trends and opportunities for improvement * Assist with implementation of corrective actions and process improvements * Support staffing coordination and continuity of care * Help ensure timely completion of clinical documentation, orders, assessments, and required follow-up * Support communication between field clinicians, clinical leadership, intake, scheduling, and operations * Participate in on-call responsibilities and provide appropriate after-hours clinical availability as needed ⭐ Additional Role: Clinical Director Designee In addition to the responsibilities above, the Clinical Director Designee serves as the agency's designated clinical leadership backup when the Clinical Director is unavailable. When functioning in the designee capacity, responsibilities may include: * Assume delegated Clinical Director responsibilities during the Clinical Director's absence * Provide clinical leadership and decision-making for day-to-day patient-care matters * Serve as the escalation point for urgent clinical issues * Support clinical staffing decisions and continuity of patient care * Provide guidance to clinical staff regarding patient-care and documentation concerns * Help ensure clinical operations continue in accordance with agency policies and applicable regulatory requirements * Communicate significant clinical, compliance, staffing, or patient-safety concerns to the Administrator and/or Clinical Director as appropriate * Exercise only those responsibilities that are within the RN's scope of practice and have been formally delegated by the agency The designee function does not replace the Clinical Director position. The employee normally performs the Clinical Supervisor-level responsibilities described above and assumes designated Clinical Director responsibilities when coverage is required. ✅ Qualifications * Current Hawaii Registered Nurse (RN) license * Graduate of an accredited nursing program * Current CPR certification * Minimum 1 year of home health experience required * Supervisory or clinical leadership experience preferred * Strong knowledge of home health clinical operations and Medicare requirements * OASIS experience * Strong clinical judgment and decision-making skills * Strong communication and leadership abilities * Ability to supervise, coach, and mentor clinical staff * Ability to prioritize competing clinical and operational responsibilities * Ambulatory care experience preferred Hands-On Clinical Leadership This is an active clinical leadership position, not an exclusively administrative role. Candidates must have: * Valid driver's license * Reliable, insured vehicle * Ability to travel throughout Oahu * Ability and willingness to perform patient visits * Ability to provide clinical and field coverage when needed * Ability to respond to clinical issues and communicate appropriately after normal business hours when necessary Ready for Your Next Step in Home Health Leadership?
