In this vital role, you'll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support. What You Bring to the Table: Essential Qualifications: This role oversees the social function of the household, including activities. This position serves as a mentor/leader and partners with the Clinical Coordinator in the coordination of care of the residents within the household. · Develops positive resident relationships, exploring approaches to aging and supporting families. · Addresses and documents any special psychological concerns (death in family, health decline etc.). · Maintains individualized psychosocial assessment. · Evaluates personal and emotional concerns. · Assists with the completion of an individualized activity assessment. · Maintains household activity calendar and coordinates activities and ensure resident appointments are kept.
