Job Details

After Hours Hospice Triage RN will work a combination of ONSITE Call Center Triage RN hours in the Bronx NY office (Registered Nurse MUST live in the NYC metro) and will also do emergency hospice patient visits when needed in the community, usually to a SNF, hospital and sometimes to the home of the hospice patient.

• RN is responsible for the emergent triaging of calls received from patients and/or caretakers for the purpose of telephonically assessing their needs in order to treat emergent situations that have developed during the Call Center hours
• Provide clinical triage and nursing assessment via phone and telehealth platforms to determine acuity, prioritize care needs, and guide appropriate next steps
• Utilize evidence-based triage protocols to assess symptoms, provide patient education, and escalate care when necessary
• Collaborate with interdisciplinary teams, caregivers, and providers to ensure continuity of care and patient safety
• Accurately document assessments, interventions, and outcomes in EHR system KanTime while maintaining HIPAA compliance
• Hospice Triage RN provides patient physical and psychosocial health systems assessments telephonically, supporting the Plan of Care, "Goals of Care" based on the patient's and caretaker's needs/wishes, and in coordination with plan developed/placed by the primary RN Case Manager for the patient.

Work Schedule: Monday thru Friday from 5 PM to 1 AM (Alternating weekends included) Qualifications: RN with active New York state RN license in good standing RN with at least 1 year experience working within the scope of an RN in either med surg, ICU, Oncology, palliative care or hospice preferred No new Grads!
