LPN Job Details

• Shift: Evening shift, 3:00 p.m.–11:30 p.m.

• Weekend/Holiday Requirement: Every other weekend and holidays as scheduled
• Start Date: November 17, 2026
• Assignment Duration: Approximately 14 weeks, ending February 20, 2027
• Work Environment: On-site, long-term care setting

Licensed Practical Nurse Job Requirements

• Current New York LPN licensure
• Current Basic Life Support (BLS) certification through the American Heart Association or RQI
• At least 1 year of experience working as an LPN
• At least 1 year of experience providing care in a long-term care facility preferred
• Ability to provide safe, age-appropriate care to adult and geriatric residents
• Strong communication, clinical judgment, organization, and teamwork skills
• Ability to work the required evening schedule, including every other weekend and holidays

Long-Term Care LPN Responsibilities

• Provide direct and indirect skilled nursing care to assigned residents under the direction of a Registered Nurse.
• Collect and document data related to residents’ physical and psychosocial conditions.
• Communicate resident findings and changes in condition to the RN and interdisciplinary care team.
• Contribute to individualized, developmentally appropriate, and culturally responsive plans of care.
• Implement nursing interventions and evaluate resident outcomes within the scope of LPN practice.
• Recommend updates to care plans and identify the need for additional services or referrals.
• Promote a safe, healthy, and respectful environment for residents and their families.
• Maintain clinical knowledge and competency through continuing education and in-service programs.
• Collaborate with nursing staff, physicians, ancillary services, residents, families, and other members of the interdisciplinary team.
• Participate in staff mentoring, orientation, and quality-focused nursing practices as assigned.
