Work Environment

This position is strictly onsite within a school facility in Houston, TX, providing a collaborative and supportive atmosphere for both staff and students. As a full-time employee, you will work an average of 40 hours per week, generally from 9:00 a.m. to 6:00 p.m. with occasional evening clinic hours and weekend community outreach events. This role offers a unique opportunity to work closely with children and their families, ensuring a positive impact on their health and overall well-being.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, identifying both preventative and acute health issues.
• Diagnose and manage both chronic and acute medical conditions while providing preventive care and pursuing health promotion initiatives.
• Work collaboratively with community organizations to design and implement targeted health education programs tailored to the community's needs.
• Deliver culturally competent care, ensuring that all patients are treated with respect and dignity, while advocating for their rights and access to healthcare.
• Coordinate referrals and facilitate follow-up care to ensure patients receive comprehensive and continuous medical attention.
• Maintain accurate and detailed medical records in compliance with healthcare regulations and organizational standards.
• Engage in community outreach activities and health fairs to enhance public health awareness and provide essential health screenings.
• Continuously monitor and adapt to emerging community health trends, ensuring an evidence-based approach to practice.

Patient Population / Client Focus

The primary patient population for this role consists of students and families within the school environment in Houston. This diverse client base may include children from various socioeconomic backgrounds and cultural identities, requiring a sensitive and adaptable approach to care.

Qualifications

• Must possess current Nurse Practitioner certification and an active state nursing license in Texas.
• A Master's degree in nursing from an accredited program is mandatory.
• A minimum of 3 years of clinical experience is required, with preference given to those who have worked in community health or public health settings.
• Excellent interpersonal and communication skills are essential, as you will be working with a wide range of patients and stakeholders.

Compensation and Benefits

This position offers a competitive salary range of $120,000 to $220,000 annually, dependent on experience and qualifications.
