Eldercare Nurse Clinic Manager

Job Description Summary Position responsible for managing the Eldercare PACE Model of Care, ensuring clinical excellence, compliance, and efficient operations. Responsibilities In collaboration with department leadership, reviews and revises department policies/procedures as necessary. Ensures the appropriate distribution and education of department policies/procedures. Manages the data collection, aggregation and dissemination across the continuum for clinical, financial and operational performance. Conducts concurrent and retrospective chart audits as necessary. Maintains and monitors daily reports to ensure appropriate staffing levels based on unit of service standards. Oversees daily coordination and implementation of the Eldercare PACE Model of Care. Directs, supervises, hires, fires and evaluates the performance of direct reports, including clinical oversight and competency evaluations of Center C.N.A.’s. Maintains stability and reputation of Eldercare by ensuring compliance with local, state, federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements. Assists in program development and implementation of Eldercare's strategic plan, including short and long-term goals and budgets. Assigns or designee to interdisciplinary team meetings, communicating participant changes and collaborating on care planning decisions. Oversees and directs coordination of home health, rehabilitative, long-term care and medical/specialty services according to participant care plans. Manages clinic flow, oversees appointment bookings, triaging and gatekeeping in conjunction with primary care and staff. Ensures timely and appropriate care of participants. Assists Director with evaluation of Eldercare's performance against national results and established service standards, clinical outcomes, utilization and financial criteria. Establishes and maintains positive working relationships with all members of the Eldercare team, hospital, and Network Providers. Oversees signing off orders, ordering medications from pharmacy, disseminating orders to appropriate personnel and ensuring physician orders are implemented. Communicates with weekend and after-hour staff, following up appropriately; participates in on-call rotation. Promotes effective communication and collaboration between the clinic and enrollment, activity and business office members. Participates in Leadership meetings to review risk scoring, pharmacy benefit manager Part D reviews, utilization, QI directives, KPI reviews and other metrics at least monthly; addresses concerns with Staff Director. Improves review processes and updates them as needed. Conducts in-person comprehensive initial and regularly scheduled assessments and other nursing assessments of Eldercare enrollees, either directly or via delegation and oversight. Performs or supervises general skilled nursing care to participants. Performs or supervises administration of prescribed medications and treatments according to nursing standards. Assists treatment, examination and testing of participants. Assists with all aspects of hospital, SNF or ALF admissions. Coordinates participant care with outside contracted service providers (nursing homes, assisted living, lab, oxygen, etc.). Identifies and develops action plans to improve Eldercare operations related to financial performance of PACE in collaboration with the director, interdisciplinary team, enrollment and MSMC departments. Identifies opportunities and strategies to improve department processes to meet evolving participant, Eldercare and community needs. Monitors and reports department quality, service and cost outcomes. Supports the director in implementing objectives and recommends improvement that support the vision and mission of the organization. Demonstrates awareness and support of department objectives during interactions with staff and customers. Partners in planning and conducting meetings, encouraging open communication and identifying and resolving departmental problems. Participates in hiring, reviews, competencies, discipline, and grievance handling to ensure optimal performance. Participates in staff development activities and interplays with director, customers to continuously improve competency and effectiveness. Identifies and addresses skill or behavior needs for improvement at individual and department levels. Provides coaching to optimize performance and ensures customer service. Engages in self-development in professional and managerial areas; completes leadership development curriculum. Adheres to standards, policies and procedures at Eldercare, hospital and departmental level. Promotes a service orientation in performance of responsibilities and interactions with participants, staff and visitors. Qualifications Current RN Licensure in the State of Florida. BLS or CPR and ACLS required. Graduated from an accredited school of registered nursing (BSN preferred). Minimum three (3) years of health care experience, preferable in a geriatric physician office and required in a nurse management role in managed care, hospital, nursing home or urgent care. Minimum one year of experience working with frail or elderly population required. Training or related experience in direction of a health care program.
