What You'll Be Doing Provide skilled speech therapy services in patients' homes Complete speech-language pathology evaluations and assessments Develop individualized treatment plans Address dysphagia, cognitive-communication deficits, speech impairments, language disorders, and voice disorders Educate patients, caregivers, and family members Collaborate with nurses, physical therapists, occupational therapists, and case managers Maintain timely and accurate Medicare-compliant documentation Qualifications Current Hawaii Speech-Language Pathologist license Certificate of Clinical Competence (CCC-SLP) preferred Minimum one year of clinical experience Home health experience preferred Strong communication and organizational skills Ability to work independently in the field Reliable transportation and valid driver's license #J-18808-Ljbffr
