LVN / LPN - Ambulatory Care/Clinic/Outpatient

Job Description

Job Summary:
Provide high-quality nursing care in a fast-paced outpatient family medicine setting, supporting physicians and ensuring patient needs are met efficiently. This role requires autonomous work, strong clinical skills, and the ability to adapt to varying patient and departmental demands.

Responsibilities:

• Perform patient assessments, vital signs, and clinical documentation accurately within the EPIC system.
• Administer injections, perform EKGs, blood pressure checks, ear flushes, and collect nasopharyngeal swabs as appropriate.
• Float between multiple outpatient locations, providing consistent, reliable care across diverse settings.
• Maintain compliance with travel policies, licensing requirements, and billing modules, ensuring accurate and timely documentation.
• Communicate effectively with patients and team members to coordinate care and address patient concerns promptly.
• Adhere to all clinical policies, including background checks, charting, and module completion requirements.

Qualifications:

• Valid NY LPN license in hand required for submission.
• Minimum of 1 year of outpatient clinical experience, preferably in family medicine or pediatrics.
• Proficiency with EPIC charting and documentation.
• Ability to work independently and handle multiple responsibilities in a fast-paced environment.
• Strong communication skills, dependability, and punctuality.
• Flexibility to float between locations and work within designated shifts.
