RN - Care Manager

Registered Nurse (RN)

We are seeking a dedicated and skilled Registered Nurse (RN) with a valid Oregon or compact license to join our team. The position requires a commitment to providing excellent care and requires flexibility to work weekends and holidays, including a rotating schedule from Sunday through Saturday.

Responsibilities

• Provide case management and patient care coordination, utilizing at least one year of relevant experience.
• Participate in patient assessments, develop care plans, and ensure quality care delivery.
• Utilize EPIC charting and maintain accurate patient records.
• Support patients covered by Medicaid and Medicare, ensuring compliance with all applicable regulations.
• Float to at least one neighboring location as necessary to meet patient care needs.

Qualifications

• Active Oregon or compact Registered Nurse license must be on file.
• A minimum of one year of experience in case management or patient care coordination.
• Prior experience with EPIC charting is preferred.
• Knowledge and experience working with Medicaid and Medicare systems are required.

This position does not require an on-call component. Candidates should be prepared to work across multiple locations within the region as necessary.
