Pathways Health Partners seeks a Case Manager – Nurse (RN) to round on hospitalized patients, coordinate care with bedside staff and treating providers, and assess post-discharge needs. The role includes documenting assessments for post-acute care across inpatient, ER and observation units. The ideal candidate holds an RN license in Florida (BSN preferred) with 2+ years in case management.
