365 Healthcare is recruiting a Board Certified or Board Eligible Endocrinology Physician for a full-time permanent opportunity in Albany, New York. Position Snapshot

• Base Salary: $245,000 – $260,000 + productivity model
• Sign-On Bonus: $35,000
• Retention Bonus: $15,000 (over 2 years)
• Schedule: Primarily outpatient, weekday schedule
• Call: 1:10 rotation
• Employment Type: Full-time, permanent

Schedule & Call Structure

• Primarily outpatient practice
• Flexible weekday scheduling
• Call: Approximately 1:10
  • Additional inpatient coverage only when inpatient providers are off
• Balanced model with minimal disruption to outpatient workflow

Practice Environment

• Established endocrinology group with strong referral base
• Multidisciplinary team including:
  • Endocrinologists
  • Advanced Practice Providers
  • Certified Diabetes Educators
• Focus on diabetes, endocrine, and metabolic disorders
• Access to advanced diabetes technology including:
  • Continuous glucose monitoring (CGM)
  • Insulin pump management
• Epic EMR

This is a patient-centered, innovation-driven practice focused on long-term disease management and outcomes.

Clinical Responsibilities

• Diagnose and manage endocrine and metabolic disorders
• Provide comprehensive diabetes care and management
• Utilize advanced technologies for patient monitoring and treatment
• Perform outpatient evaluations and follow-ups
• Collaborate with multidisciplinary care teams
• Deliver patient education and long-term disease management strategies

Qualifications

• Board Certified or Board Eligible in Endocrinology
• Active New York license (or ability to obtain)
• Strong interest in outpatient endocrine care
• Clean malpractice and license history

Open to experienced physicians and upcoming/recent graduates. Open to H-1B visa candidates (no J-1 waiver support).

Compensation & Benefits

• $245K–$260K base + productivity incentives
