CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. Physical Therapist (PT) - Full-Time

Assisted Living | Independent Living | Home Care & Outpatient

Love building relationships with your patients instead of treating someone new every hour? Community Physical Therapy (CPT) is looking for a compassionate Physical Therapist to join our growing Continuum of Care team.

This unique position combines home care and outpatient therapy within Assisted Living and Independent Living communities, allowing you to provide individualized care while helping seniors maintain their independence and quality of life.

What You'll Do

Provide skilled physical therapy services in Assisted Living and Independent Living communities

Deliver both home care and outpatient therapy services

Develop individualized treatment plans focused on functional outcomes

Complete timely documentation and required administrative tasks

Collaborate with an interdisciplinary team to provide exceptional patient care

Build lasting relationships with residents, families, and care partners
