Chestnut Health is a leading multi-specialty healthcare organization dedicated to providing the entire continuum of rehabilitative care. Our expert team specializes in Physical Medicine and Rehabilitation (PM&R) - Physiatry, focusing on restoring function and improving the quality of life for individuals with injuries, illnesses, or disabilities.

We are seeking a clinical professional to lead outbound facility engagement across Skilled Nursing Facilities (SNFs). In this role, you will connect directly with key SNF decision-makers including Administrators, Directors of Rehab (DORs), and Directors of Nursing (DONs) to introduce our specialized Physiatry programs.

Using a blend of warm follow-ups and strategic cold outreach over the phone, you will educate facility leadership on how integrating specialized physiatry enhances functional outcomes, optimizes therapy throughput, and reduces hospital readmissions.

Key Responsibilities

• Multi-Channel Phone Outreach: Conduct outbound calls to warm and cold targeted facility leads across designated SNF territories to reach decision-makers (NHA, DON, DOR).
• Peer-to-Peer Consultation: Speak clinician-to-clinician with facility leadership to identify operational pain points and introduce our Physiatry program benefits.
• Meeting Discovery & Scheduling: Qualify facility fit and schedule virtual (Zoom) presentations between SNF leadership and our practice leaders.
• Pipeline Management in HubSpot: Log all activity, track outreach sequences, update contact lifecycles, and move opportunities through our HubSpot CRM sales pipeline.
• Contract Onboarding: Educate decision-makers on the seamless integration of our providers and facilitate the execution of Business Associate Agreements (BAAs).

Requirements

Key Qualifications

• Clinical Background (Required): Active or past credentials/licensure as a PT, OT, PTA, or COTA.
• SNF Expertise: Direct experience working within or closely alongside Skilled Nursing Facilities, sub-acute rehabilitation, or post-acute clinical teams.
• Phone Confidence & Resilience: Comfortable managing phone outreach, navigating gatekeepers, and building fast rapport over the phone.
• CRM & Tech Familiarity: Experience using HubSpot CRM (or similar CRM platforms) to log activities, track leads, and manage tasks, along with comfort using Zoom.
• Sales Training Provided: Prior formal sales experience is not required —we will train you on outreach workflows, messaging, and contract navigation.
