As a PMHNP on our Intensive Case Management Team, you'll care for individuals experiencing serious mental illness, substance use disorders, homelessness, and complex medical needs. Working within an integrated, multidisciplinary model, you'll collaborate alongside therapists, case managers, primary care providers, addiction specialists, and peers to deliver compassionate, trauma-informed care that meets clients where they are.

What You'll Do

• Provide psychiatric evaluations, diagnosis, medication management, and ongoing treatment
• Prescribe buprenorphine, Vivitrol, Naltrexone, Acamprosate, and other medications supporting recovery
• Deliver brief psychotherapy and motivational interviewing interventions
• Respond to psychiatric crises and provide suicide risk assessment and intervention
• Partner closely with behavioral health, primary care, and substance use treatment teams
• Help shape individualized treatment plans for some of Portland's most vulnerable community members

Old Town Recovery Center Recovery Center provides treatment for acute and chronic health concerns/primary care, mental health support, care for substance use disorders and an on-site pharmacy. The Psychiatric Mental Health Nurse Practitioner (PMHNP) works collaboratively as a member of a multi-disciplinary team providing outpatient SUDS and Mental Health services.

Schedule: Monday-Thursday 8:30am-5:00pm

Location: Old Town Recovery Center Compensation: $164,000-$213,000/annually *Range varies based on professional experience and licensure/certifications

Minimum Qualifications:

• Bachelor's degree in nursing and Master's degree in Psychiatric/Mental Health Nursing from State Board of Nursing approved schools of nursing.
• Current license to practice nursing in Oregon and certified as a PMHNP by the Oregon State Board of Nursing and National certification.
• Must have full prescriptive powers in Oregon with a DEA number.
• Must meet CCC privileging requirements as required by FTCA.
• Current BLS certification required prior to start.
• Bilingual in Spanish preferred.
• Must meet applicable training requirements for Buprenorphine prescribing.
• Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
• Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
• The OAR prohibits SUDS program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
• Must be able to work within an integrated, multidisciplinary setting.
• Must adhere to agency's non-discrimination policies.
• Must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy.? Must pass an initial driver training within 60 days of being an approved driver and continued recertification training.
• Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988.?An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.

Essential duties and responsibilities:

• Work as a member of the Old Town Recovery Center multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
• Actively participate in the integration of services at Old Town Recovery Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
• Proactively identify opportunities for improvement within Old Town Recovery Center workflows and logistics. Participate in review of Old Town Recovery Center Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
• Build understanding of and appreciation for diversity among Old Town Recovery Center clients and support their connection to culturally responsive and culturally specific services as requested.
• Provide direct patient care including but not limited to: psychiatric evaluation, consultation, psychopharmacological and psychotherapeutic interventions, and monitoring and addressing adverse effects.
• Prescribe and manage the opioid agonist medication, buprenorphine, as well as other medication supported recovery medications such as Vivitrol, Naltrexone, and Acamprosate.
• Document plan for medical and psychiatric treatment and on-going treatment in the outpatient setting.
• Provide crisis intervention, suicide assessment and intervention.
• Conduct care of psychiatric/mental health emergencies and initiate arrangements for continuing care or referral to appropriate facility.
• Provide brief psychotherapy interventions including motivational interviewing.
• Provide patient and family education.
• Provide clinical consultation to staff in behavioral and primary care disciplines, including consultation to other CCC SUDS practitioners.
• Participate in clinical staff meetings and case conferences in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients.
• Refer patients, as necessary, to other mental health care providers and community resources/services.
• Participate in records review to ensure that services are clinically appropriate and effective.
• Maintain client records as required by program standards and funding requirements.
• Participate in compliance, quality assurance and quality improvement activities as directed.
• Participate in training opportunities to advance skills and knowledge in behavioral and primary care.
• Work within scope of training and experience and seek consultation when appropriate.
• Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.? Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access.? This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
• Attend all mandatory CCC trainings in a timely manner.
• Perform other duties as assigned.
