Nurse Practitioner- PRN

Role Overview Matrix Medical Network is seeking a compassionate and adaptable Nurse Practitioner – PRN to join our Healthcare & Medical Services team. In this flexible, as‑needed role, you will deliver high‑quality, patient‑centered care through in‑home health assessments and related clinical services for health plan members and other populations we serve. Responsibilities Work independently in the field, traveling to patients’ homes to perform comprehensive health evaluations, including detailed medical histories, physical examinations, medication reviews, and screenings aligned with evidence‑based guidelines. Identify care gaps, evaluate functional status and social determinants of health, and provide education that empowers patients. Document thoroughly in electronic health records to support quality initiatives, care coordination, and compliance with payer and regulatory requirements. Collaborate closely with clinical and quality teams, communicating findings and escalating concerns when needed. Order or recommend appropriate diagnostic tests within your scope of practice, review and interpret results, and provide clear follow‑up recommendations to primary care providers and health plan partners. Practice according to state practice acts, organizational protocols, and the highest standards of patient safety and infection control. Assist in quality and process‑improvement efforts and adopt new tools and technologies that enhance the patient experience. Qualifications Strong clinical assessment skills, sound judgment, and excellent communication abilities. Comfortable working independently in the community, managing a diverse panel of patients, and adjusting to changing program requirements. Experience in primary care, geriatrics, home health, or community‑based medicine is highly desirable. Proficiency in delivering patient‑centered care in home settings and a willingness to work a flexible PRN schedule.
