Responsibilities:
- Provide high-quality patient care to a diverse population of residents, including sub-acute rehabilitation and long-term care patients
- Develop and implement individualized care plans, ensuring patient-centered care is delivered
- Collaborate with interdisciplinary teams to ensure seamless communication and effective patient care
- Monitor and record patient vital signs, medication administration, and treatment plans
- Educate patients and families on treatment plans, self-care techniques, and disease management
- Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction
Requirements:
- Current Registered Nurse (RN) licensure in Maryland
- Registered Nurse (RN) experience preferred
