- Conduct assessments for patients in both inpatient and outpatient settings and craft personalized treatment plans
- Deliver physical therapy interventions that emphasize mobility, strength, balance, and overall function
- Engage in collaborative efforts with nurses, physicians, case managers, and rehab professionals to optimize patient outcomes
- Provide education to patients and families regarding therapeutic objectives, discharge procedures, and home exercise regimens
- Maintain thorough documentation of evaluations, treatments, and patient progress aligned with facility and regulatory standards
- Actively participate in care team meetings and interdisciplinary conferences to ensure smooth coordination of care
- Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (e.g., neurological or geriatric) are a bonus but not mandatory
- Prior experience in clinical settings is highly preferred
- Proficient in treating patients with complex medical and mobility challenges
- Exemplary communication and teamwork abilities
- Familiarity with digital charting and clinical documentation systems
