Description
  • Conduct assessments for patients in both inpatient and outpatient settings and craft personalized treatment plans
  • Deliver physical therapy interventions that emphasize mobility, strength, balance, and overall function
  • Engage in collaborative efforts with nurses, physicians, case managers, and rehab professionals to optimize patient outcomes
  • Provide education to patients and families regarding therapeutic objectives, discharge procedures, and home exercise regimens
  • Maintain thorough documentation of evaluations, treatments, and patient progress aligned with facility and regulatory standards
  • Actively participate in care team meetings and interdisciplinary conferences to ensure smooth coordination of care
Education
  • Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
Certifications
  • Current state licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional specialized certifications (e.g., neurological or geriatric) are a bonus but not mandatory
Skills
  • Prior experience in clinical settings is highly preferred
  • Proficient in treating patients with complex medical and mobility challenges
