SCHEDULE
• Coverage Type: Inpatient
• Schedule Type: Inpatient
• Call Type: None
• Dates of Need: September 8, 2026, to September 21, 2026
• Shift Times: 6am-6pm and 6pm-6am
• Lunch: N/A
• Provider Min Shifts Required: One provider for all shifts below
• Guaranteed Hours per Day: 12
• Hours Worked for OT to apply: 12 per day
• Rates to be Filled Out on Bid: Regular, orientation, Overtime, Holiday


CREDENTIALING
• Current CV (must be uploaded to bid)
• License: Active WA or IMLC
• DEA Required: Yes
• Boards: BC only
• Skills/Certs: N/A


ADDITIONAL DETAILS
We are looking to add one locum to our locum pool who can cover the following dates:
• Holy Family, days Sept 8-21 (6am-6pm). Coverage is needed for the entire two-week stretch.
• Sacred Heart nights, Oct 9-12 (starting 6pm and finishing by 6am on Oct 13).
• SHMC nights Dec 22-28 (starting 6pm, finishing at 6am on Dec 29) OR Holy Family, days Dec 22-28 (6am-6pm). Scheduled coverage will be determined by provider’s preference for days or night shifts.


Hourly Pay Rate: $1.00
