Position Summary:We are actively seeking an experienced Travel Registered Nurse (RN) – Case Manager to join a highly respected acute care hospital in Mission Viejo, CA. This 13-week travel assignment is ideal for an RN with recent acute care case management experience who excels in care coordination, discharge planning, and multidisciplinary collaboration. Key Responsibilities:

• Coordinate patient care throughout hospitalization.
• Perform comprehensive patient assessments.
• Develop and implement individualized discharge plans.
• Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
• Arrange DME, SNF placement, home health services, and transportation.
• Participate in family meetings and treatment planning.
• Ensure compliance with CMS, HIPAA, Joint Commission, and hospital regulations.

Requirements:

• Active California RN license
• BLS by AHA
• COVID card (Medical/Religious exemptions are accepted)
• Minimum 2 years of recent Acute Care RN Case Management experience
• Recent Care Coordination experience within the last 3 months
• Previous Travel Nursing experience  required
• Epic EMR experience  required
• Experience with discharge planning and care coordination in an acute hospital setting
