Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse, Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Coordinate patient care through effective discharge planning and needs assessment.
• Organize and participate in family meetings for patient assessments and planning.
• Participate in developing and implementing individualized treatment plans.
• Arrange necessary Durable Medical Equipment (DME) and coordinate transports as needed.
• Provide competent care coordination for patients with diverse needs, including mental health and substance abuse.
• Float to the Laguna Facility as required to support staffing needs.

Qualifications

• Current and unrestricted RN license in California, or pending license to be completed by start date.
• Minimum of 2 years of acute medical care management experience with care coordination.
• Proficiency in Epic charting system and experience with computerized documentation.
• Weekend rotation availability and flexibility for floating as needed.
• Comprehensive understanding of regulatory and compliance requirements, including HIPAA and OSHA.
• Prior travel experience required; first-time travelers not accepted.
