How you’ll make an impact in this role

• Coordinate holistic care plans by aligning multidisciplinary teams around each patient’s unique treatment goals to ensure seamless, well-managed care journeys.

• Monitor and respond to critical condition changes, communicating real-time updates to care teams to ensure prompt intervention and high-quality outcomes.

• Execute precision treatment plans in alignment with physician orders and safety protocols to maintain high standards of patient safety and clinical excellence.

• Guide smooth care transitions by assessing and managing discharge needs early, preparing patients and families for a safe and confident return home.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
