Job Summary:

Provide expert physical therapy services to patients with various injuries and conditions impacting mobility and independence. Assess, develop, and implement personalized treatment plans to optimize recovery and functional outcomes.

Responsibilities:

• 
  Evaluate patients physical conditions and establish accurate diagnoses.
  
  
• 
  Develop and execute individualized treatment plans aligned with patient needs.
  
  
• 
  Monitor patient progress and adjust therapy strategies accordingly.
  
  
• 
  Educate patients and caregivers on exercises and techniques to promote healing.
  
  
• 
  Document treatment sessions and maintain detailed patient records.
  
  
• 
  Collaborate with multidisciplinary teams to ensure comprehensive care.
  
  

Qualifications:

• 
  Hold a current, unencumbered Oregon Physical Therapy license.
  
  
• 
  Possess Healthcare Provider Level BLS certification.
  
  
• 
  Demonstrate ability to travel between multiple care locations.
  
  
• 
  Show strong knowledge of rehabilitative therapy techniques and modalities.
  
  
• 
  Exhibit excellent communication, interpersonal, and documentation skills.
