The Physician Assistant or Nurse Practitioner is a licensed professional provider who functions in collaboration with an attending physician in a variety of practice settings at YNHHS. Provides exceptional direct diagnostic and therapeutic patient care to the highest standards of the organization as part of a multidisciplinary team. Cultivates a patient centric environment which focuses on the whole individual inclusive of physical, psychosocial, spiritual and functional needs of the patient, family, and significant others. Serves as the focal professional for the continuity of care in patient care within scope of practice standards, as well as specialty specific standards to address and resolve problems that may arise.

Responsibilities

Clinical Care

• Provide comprehensive assessment, triage, diagnosis, treatment, and ongoing management of patients across inpatient, ambulatory, and outpatient settings.
• Collaborate with supervising/collaborating physicians and interdisciplinary care teams to deliver high-quality, patient-centered care in accordance with state regulations, hospital policies, clinical protocols, and professional standards.
• Obtain medical histories, perform physical examinations, order and interpret diagnostic and therapeutic tests, and develop individualized treatment plans.
• Prescribe medications and treatments consistent with state licensure, institutional guidelines, specialty practice standards, and professional competencies.
• Perform clinical procedures within scope of practice and demonstrated competency.
• In applicable specialties, serve as a first or second assistant during surgical procedures utilizing appropriate technical and manual skills.
• Respond to urgent and emergent patient care situations as required.
• Participate in autonomous or team-based management of designated patient populations as appropriate to specialty and clinical setting.

Patient Education and Support

• Educate patients and families regarding diagnoses, treatment plans, medications, procedures, potential side effects, discharge instructions, and available resources.
• Provide compassionate psychosocial support to patients, families, and caregivers as part of a multidisciplinary healthcare team.
• Promote patient understanding and engagement to support informed decision-making and optimal health outcomes.

Documentation and Information Management

• Maintain accurate, timely, and complete clinical documentation in compliance with hospital, regulatory, and accreditation standards.
• Ensure the confidentiality, integrity, and security of patient information across all forms of communication and documentation.
• Utilize electronic medical record systems and other healthcare technologies effectively to support patient care.

Quality and Compliance

• Demonstrate professionalism, ethical conduct, compassion, accountability, and commitment to excellence in patient care.
• Adhere to all applicable healthcare regulations, accreditation requirements, organizational policies, and quality standards.
• Participate in quality improvement initiatives aimed at enhancing patient safety, outcomes, and operational effectiveness.
• Consistently demonstrate high-reliability behaviors, including reporting safety events, near misses, and opportunities for process improvement.

Professional Development and Leadership

• Maintain clinical expertise through ongoing professional education, certification requirements, and competency development.
• Serve as a clinical resource for physicians, advanced practice providers, nursing staff, trainees, and other healthcare professionals.
• Participate in orientation and education programs for residents, fellows, and new clinical staff as appropriate.
• Act as a preceptor and mentor for NP/PA students, pre-NP/PA candidates, and other healthcare learners.
• Contribute to research, quality assurance, performance improvement initiatives, and the development of clinical protocols and best practices.
• Maintain all required licensure, certifications, training, and employee compliance requirements in accordance with organizational policy.

Qualifications

Education

• Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program required.

Experience

• Previous clinical experience as a Nurse Practitioner or Physician Assistant within the applicable specialty or service line is preferred; new graduates may be considered.
• Advanced Practice Registered Nurses (APRNs), Physician Assistants (PAs), Certified Registered Nurse Anesthetists (CRNAs), and Certified Nurse Midwives (CNMs) must be eligible for appointment to the Affiliated Medical Staff of a Yale New Haven Health System (YNHHS) hospital and successfully complete the medical staff credentialing process.
• Demonstrated current clinical competence through professional references from physicians and/or advanced practice providers is required and must be obtainable during the credentialing and medical staff appointment process.

Licensure and Certification

For Physician Assistant Candidates

• Current certification through the National Commission on Certification of Physician Assistants (NCCPA) required.
• Current Connecticut Physician Assistant License required.
• Basic Life Support (BLS) and/or Advanced Cardiovascular Life Support (ACLS) certification required, as applicable to the role.
• DEA Registration required, if applicable to position responsibilities.
• Pediatric Advanced Life Support (PALS) certification required for designated pediatric positions.
• Neonatal Resuscitation Program (NRP) certification required for designated neonatal care positions.

For Nurse Practitioner Candidates

• Current Connecticut Registered Nurse (RN) License required.
• Current Connecticut Advanced Practice Registered Nurse (APRN) License required.
• National board certification in the applicable specialty area required.
• Basic Life Support (BLS) and/or Advanced Cardiovascular Life Support (ACLS) certification required, as applicable to the role.
• DEA Registration required, if applicable to position responsibilities.
• Pediatric Advanced Life Support (PALS) certification required for designated pediatric positions.
• Neonatal Resuscitation Program (NRP) certification required for designated neonatal care positions.

Additional Requirements

• Additional licenses, certifications, continuing education, and specialty-specific training may be required based on departmental needs, clinical specialty, and evolving regulatory requirements.
• Must maintain all applicable licensure, certifications, credentialing, and continuing education requirements throughout employment.
